Admissions
Columbus West Park accepts admissions 24 hours per day, 7 days per week. Tours are available throughout the week from 8am-8pm. If you would like to schedule an appointment with our Admissions Director for a tour, please call (614) 274-4222.
The admissions procedures are slightly different depending on what setting you or your loved one will be coming from, and are explained below. Some aspects of the admissions process are the same:
- Upon admission, our nursing staff will complete a head to toe assessment, and develop a plan of care unique to each resident.
- Other departments, such as therapy, dietary, activities, and social services will meet with the new resident to gather information such as rehabilitative needs, meal preferences, etc.
- Our Admission Director will meet with the resident or responsible party to identify key contact information, copy Medicare and/or insurance cards, gather copies of any advance directives, and go over other admissions paperwork, including the consent to treat.
- Residents admitting for rehabilitative stays will be contacted soon after admission to have a care conference scheduled with our interdisciplinary team. The intent of this meeting is discuss the plan of care and establish outcome goals
Admission from Hospital
If you or your loved one are currently in the hospital, or planning for a surgery, and will need either a short term rehabilitative stay or long term placement, Columbus West Park can meet your needs. Social Workers, also known as Discharge Planners are available in the hospital to assist you. We wanted to share some basic information about our procedures.
- To qualify for a Medicare skilled stay, you must have a 3-day qualifying stay in the hospital.
- You need to let the Discharge Planner know that you are interested in placement at Columbus West Park.
- The Discharger Planner will fax over medical information for our team to review. We evaluate on a case by case basis to ensure that we can meet the person’s needs and provide the level of care required
- Our Admissions Director will work with the Discharge Planner to determine the date and time of transfer, and to make sure that everything is set up for the admission
Transfer from Home or Assisted Living Facility
If you or your loved one are currently at home or in an independent or assisted living environment and you need a higher level of care, Columbus West Park can accommodate a short term respite stay or long term placement. The following are our procedures for admission.
- Columbus West Park must have a history and physical completed by the primary care physician within 5 days prior to admission. This must include a list of current medications and their dosage. The Admission History and Physical form can be downloaded here.
- If Medicaid will be the primary payor source, the Central Ohio Area Agency on Aging (COAAA) requires that a long term care assessment be completed. You may contact COAAA’s pre-admission screening department at (614) 645-7250 to schedule the assessment.
- Our Admissions Director will work with the resident or their responsible party to determine the date and time of transfer, and to make sure that everything is set up for the admission
Transfer from Skilled Nursing Facility
If you or your loved one are currently in another skilled nursing facility and would like to transfer to Columbus West Park, you need to notify the social services/discharge planning department of your intent to transfer.
- Under the Health Information Privacy regulations you must give permission for the other skilled nursing facility to release medical records to Columbus West Park.
- Once permission has been given, the Discharger Planner will fax over medical information for our team to review. We evaluate on a case by case basis to ensure that we can meet the person’s needs and provide the level of care required.
- Our Admissions Director will work with you and the Discharge Planner to determine the date and time of transfer, and to make sure that everything is set up for the admission.
