Columbus West Park: Approved Tricare Provider

Columbus West Park Nursing and Rehabilitation Center, a 100 bed skilled nursing facility, has been an approved provider of skilled services under the Tricare program for more than 10 years. We are family owned and operated since 1992, and our owner Tom Stewart is a Navy veteran of the Vietnam War era.  We are proud to serve American Veterans and their families.  In the Fall of 2006 Columbus West Park received a certificate of recognition from The National Committee for Employer Support of the Guard and Reserve for being a Patriotic Employer for contributing to national security and protecting liberty and freedom by supporting employee participation in America’s National Guard and Reserve Force.  Administrator Sarah Mitchell was particularly recognized for her efforts to support a state tested nursing assistant during her two tours in Iraq.

We have a team of excellent medical and nursing staff to meet the needs of our residents. We are partnered with Marden Rehabilitation to offer physical, occupational and speech therapies up to 6 days per week to our residents. Our partnership with Dr. S. Kwon Lee, MD, FACS and his team at Physicians Wound Consultants, Inc. enables us to provide outstanding resident wound care. Columbus West Park also has contracts to provide dental, optometry, podiatry, audiological and psychological services on-site.

The private and semi-private rooms on our short term unit have fully electric beds, flat screen televisions with satellite connection and telephones.  Our long term beds are telephone and cable ready.

We are a geriatric geared facility, and the majority of our patients are over the age of 60. Our admissions committee, which includes the Administrator, Director of Nursing, and Admissions Director reviews each referral case by case, and will consider younger patients if we can safely meet their medical and psychosocial needs.
